OUTCOMES AND AFTERCARE
At Bremwood, we have a formal process for tracking and monitoring client care. Outcomes are determined by how our teens meet a percentage of their treatment goals.
After a teen is discharged from Bremwood, LSI can still offer support and services if needed. If a teen receives Medicaid insurance, he or she is eligible for our remedial services program, which helps teens build skills like anger management, coping and decision-making.
We have LPHA professionals on our Bremwood campus who can do an assessment of the teen to see if remedial services will be needed. A caseworker can then begin meeting with the teen before discharge, when he or she is on home visits, to start building that relationship for the new service.
Certain programs at Bremwood, such Exodus Exchange and Trinity Cottage, also have specific supports for teens after they leave Bremwood, such as follow-up calls or visits. With all of our programs, parents and teens are also free to contact us again with questions or for support.
